It is one of the most hidden problems and probably <em>the<\/em> condition that stops people socialising.<br \/>\nAlthough it is often associated with older adults and dementia, it can affect younger women.<\/p>\n<p><strong>Risk groups<\/strong><br \/>\nPatients who are chronically constipated and strain to open their bowels<br \/>\nAny patients with diarrhoea.<br \/>\nPatients with anal problems:<br \/>\nWomen with third- and fourth-degree tears from childbirth injury.<br \/>\nPatients with rectal or pelvic organ prolapse.<br \/>\nPatients with pelvic radiotherapy or colon surgery.<br \/>\nInflammatory Bowel Disease <a title=\"IBD\" href=\"http:\/\/cart.gesa.org.au\/membes\/files\/Consumer%20Information\/IBD%20-%20Crohns%20Colitis.pdf\" target=\"_blank\" rel=\"noopener\">IBD<\/a><br \/>\nPatients with <a href=\"https:\/\/en.wikipedia.org\/wiki\/Obstetric_fistula\">Fistula<\/a> or <a href=\"https:\/\/www.healthdirect.gov.au\/haemorrhoids-piles\">Haemorrhoids<\/a><br \/>\nPatients with Irritable Bowel Syndrome<a title=\"IBS\" href=\"http:\/\/www.gesa.org.au\/files\/editor_upload\/File\/Consumer%20Brochures\/2014\/IBS.pdf\" target=\"_blank\" rel=\"noopener\"> IBS<\/a><\/p>\n<p><strong>Childbirth<\/strong><\/p>\n<p>Incontinence is common after the use of forceps, a large baby, occipitoposterior position and a long second stage of labour.<br \/>\nOne in five women develop incontinence after vaginal delivery and 30% have structural damage that can be seen by anal endosonography.<sup><span style=\"font-size: small;\">\u00a0 <\/span><\/sup>A systematic review reported that after repair anal incontinence persisted in 15-59% of women and anal urgency affected a further 6-28%.<\/p>\n<p><strong>Surgery<\/strong><\/p>\n<p>This is the most common cause after obstetric trauma.<br \/>\nIncontinence may be inevitable after complex anal fistula surgery, or may occur as an unexpected complication after haemorrhoidectomy or surgery for chronic anal fissure.<sup><span style=\"font-size: small;\">\u00a0 <\/span><\/sup>The incidence is reducing with advances in surgical techniques.<\/p>\n<p><strong>Degeneration of the anal sphincter <\/strong><\/p>\n<p>After structural damage, this is the most common cause of sphincter dysfunction. Most cases are due to primary isolated degeneration of the smooth muscle of the internal anal sphincter. The condition is most common in middle age and affects both men and women.<\/p>\n<p><a title=\"Endo Anal Ultrasound\" href=\"http:\/\/www.northernsydneycolorectal.com.au\/procedures-treatments\/anorectal-physiology-endorectal-ultrasound\" target=\"_blank\" rel=\"noopener\">Endosonography<\/a> is a useful investigation to identify surgically correctable sphincter defects. It often shows that the anal sphincter is fibrotic and thin and resting anal pressure is low.<\/p>\n<p><a title=\"constipation\" href=\"http:\/\/www.continence.org.au\/pages\/constipation.html\" target=\"_blank\" rel=\"noopener\">Constipation<\/a><\/p>\n<p>Constipation is usually determined by the consistency of your motion. The regularity if passing bowel motions varies. Less than 3 bowel motions a week, together with hard ,small, difficult to pass motions is typical of constipation.<br \/>\nSome people who are constipated find it painful to have a bowel movement and often experience straining, bloating and the sensation of a full bowel. When they do pass a motion it may be accompanied by a small amount of blood.
